We currently work with a range of insurance companies to ensure that you can access the care you need without financial stress. Our list of accepted insurance providers is continuously expanding, so please feel free to contact our administrative team to inquire about specific plans. 

What to Expect with Direct Billing:

When you book your very first appointment online, you will have an option to fill out your insurance information in the intake form.  If you walk-in to visit our clinic, our friendly staff will assist you in understanding the billing process and check your insurance coverage. Here's a step-by-step breakdown of what you can expect:

  1. Insurance Verification: Our team will verify your insurance coverage and any eligible benefits. Please have your insurance card and necessary photo identification card ready. We will also kindly ask you fill out the consent form for direct billing. Please arrive at least 10-15 minutes before your very first appointment to complete the paperwork if you have not yet do so.

  2. Co-Payment or Deductible: Depending on your insurance plan, you may be responsible for a co-payment or deductible, which you'll be informed of during the verification process.

  3. Claim Submission: We will handle the claim submission directly to your insurance provider, saving you the trouble of paperwork. We will provide a copy of your Explanation of Benefit (EOB) once the claim is successfully submitted. 

  4. Payment: Any uncovered costs, co-payments, or deductibles can be settled on the day of your appointment.

To ensure a smooth billing process and maintain the integrity of our direct billing services, we kindly request our patients to:

  • Provide Accurate Information: Please provide accurate and up-to-date insurance information, including your policy number and any changes to your coverage.

  • Understand Your Coverage: It's important to understand your insurance policy and coverage details. While we strive to offer direct billing, coverage may vary depending on your specific plan. Here are some sample questions to ask your insurance provider.

  • Co-Payments and Deductibles: Be prepared to pay any required co-payments or deductibles on the day of your appointment.

  • Billing Inquiries: If you have any questions or concerns about your insurance, billing, or claims, please contact our administrative team for assistance.

**Please Note:**

In the event that your insurance plan is not accepted at our clinic, we provide detailed invoices and receipts for you to submit for reimbursement.

Questions to ask your insurance provider

  4. What are my maximums for each category?

  5. Up to what amount is covered per each visit? Is it different for initial visits versus follow-ups?

  6. Do I need a referral from an MD to obtain any of these services?

  7. Do I have separate "health spending account" or "fitness account" that can be used for other health care services not listed above?
